We strive to provide the finest vision care at a reasonable cost. We participate in many health insurance plans. We encourage you to contact your insurance plan directly to make sure we are a participating provider in your plan. Insurance plans vary, so it is important you check with your insurance for confirmation of coverage and your responsibility.
If your plan requires a referral prior to your visit, we must receive it before your appointment. For those insurance plans that we are not providers for, we will provide you with the information you need to file your claim. We do require you to update your registration information and we will make a copy of your insurance card(s) each time you visit our office.
Please be aware that if you scheduled your appointment for a routine vision exam and a medical diagnosis was determined, we will bill your medical insurance primary. If there is a balance after the medical insurance pays, we will submit to your vision plan as a secondary.
Refraction testing (checking for glasses) may not be covered by your insurance company and will therefore be the patient’s responsibility at the time of service. Please be advised that to determine your specific eyeglass prescription, a refraction may be necessary. Refractions ARE NOT COVERED by Medicare.
The Practice accepts Medicare assignment. We will file all claims with your Medicare carrier. We will also file any claims to your secondary carrier, so please make sure you provide our office with your complete and current insurance information.
You will be responsible for any co-payments and non-covered charges at the time of service. Proof of insurance is expected at the time services are received. We accept cash, personal checks, MasterCard, Visa, American Express and Discover. If you have any questions regarding your bill, please contact our billing office by calling (440) 808-8341, or you send a message through our patient portal.
Please contact your insurance company prior to your appointment to find our your contractual amount for co-pays and refractions. If you have any questions, please contact our office so we may assist you.
